Bolt Failure while Torquing



My bolts fails at Head portion during torquing. It is Zinc plated & having hardness 40 HRC. Please advice what should I do? Is it require H2 De-embrittlement?

I am not an expert!
It is customary to bake any part that is 40 Rockwell.
Isolate the problem. Take say 20 random bolts before plate, 20 random bolts after plate and 20 random bolts after a H2 elimination bake that goes into the oven immediately after the last rinse.
Torque all 60 to failure and record the torque of each. The location of the problem should be obvious from the results.
