Why did my freshly painted steel door peel?

I recently sanded and repainted my 20 year old steel door. One week later I looked at it and noticed it had a lift spot in the paint. I took the piece of paint and peeled off about 8 inches. This was a very good quality of Latex semi gloss paint. What did I do wrong?

Do you know if the door was galvanized, Pamela? Galvanized metal is very difficult to paint, requiring at the very minimum a primer designed for galvanized metal.
