Pitting and Uneven on silver plating

I have pitting and uneven surface on my silver plating. I am using SPC in the Ag bath and control silver thickness about 150 microinch. Does anyone knows what are actually causing the problems? Before the Ag plating, there is anti-immersion treatment bath.
