Looking for form for supplier audit

I will have to do a survey of a company that may paint parts for us.Does anyone have or know of a standard form that I can use.

The Military loves forms. Contact a local firm that does a lot of military work and try to get a sanitized copy from their QC.

Alternative would be a larger ISO 9001 Company. They require paper work to back up the paperwork.
